You may use this form to request an appointment or a phone call from me, if you prefer. Your name, phone number,  email address, and a brief explanation (message) are required fields. I will do my best to get back to you within 24 hours of receiving your request, if received during business hours. At this time, appointments cannot be scheduled online.

Do you take my insurance?

- I do not do business with insurance companies. My practice is private pay only. There are multiple reasons, which I would be happy to discuss if you are interested.

Do you do late or weekend appointments?

- I see clients between 10AM and 5PM Monday through Thursday and between 10AM and 2PM on Fridays.
